Problem solving in Endodontics

This presentation is to share my experiences with Endodontics since my MSc in 1995. The challenges I have faced with diagnosis, how I do my consent forms, how to achieve local anaesthesia, the challenges I faced with every step of the root canal treatment, how to explain/manage post op pain/failures. Share how and why I approach Endodontics in a certain way and hopefully you can bring back some pointers to make your journey with Endodontics a little easier.

Day 1 Lecture with Dr. Koh Eng Tiong

Lecture

(Part One)ย  Etiology, Diagnosis & Management of Endodontic Pain

Participants will learn:

  • the physiology of pain and its management
  • how to diagnose pulpal and periapical infection, distinguishing difference between reversible vs irreversible pulpitis.
  • the most effective treatment option for endodontic emergencies: pulpotomy vs pulpectomy.
  • vital pulp therapy vs root canal treatment for managing irreversible pulpitis.
  • how to manage & what to prescribe for the following clinical condition:
    1. symptomatic irreversible pulpitis
    2. symptomatic apical periodontitis
    3. endodontic abscess
    4. post operation pain

(Part Two) Root Canal Therapy

Participants will learn:

  • how to manage cracked tooth
    • when to do elective crown
    • when to do vital pulp therapy
    • know how to explain to patient why long term prognosis for cracked teeth with irreversible pulpitis or periapical periodontitis are guarded/poor.
  • when to indicate for vital pulp therapy for decayed teeth
  • how to give effective local anaesthesia (including IDN block)
  • how to do rubber dam isolation and get good seal
  • how to avoid/reduce risk of perforation (lateral perforation or floor of pulp chamber) during access cavity
  • the difference in working length between treating vital and non-vital teeth
  • where do we apically stop our instrumentation, do we need to have a minimum apical size
  • the need for infection control in vital cases and dis-infection protocol in non-vital cases
  • why there is a difference in success rate between vital and non-vital teeth/with PA lesion
  • the difference between hand and rotary instrumentation
  • how to minimize file breakage during rotary instrumentation (understand why cyclic fatigue, torsional stress and micro cracks on the file surface are the main causes of file breakage. why combination of hand and rotary instrumentation is the most effective way to prevent file breakage)
  • traditional rotary tapered files (.02, .04, .06, multiple tapers etc) vs engine driven โ€“ adapting to canal anatomy type of files (SAF system, FKG XP-endo Shaper etc)
  • how new materials, products, tools and machines can help in root canal treatment
  • a instrumentation protocol with chemicals so that we can achieve biological objectives and at the same time, reduce micro-cracks and file breakage
  • ultrasonics, sonics. Metal vs plastic tips
  • irrigants and the different delivery systems, the significance of the air bubble in the root canal and how to remove it. Plastic vs metal delivery systems
  • the purpose of root fill, how to prevent underfill or overfill. The effectiveness of touch & heat or system B systems to section gutta percha
  • bioceramic sealers, while they have several advantages over traditional sealers, they also have some disadvantages.
  • single visit vs multiple visit RCT
  • trauma cases, age and A- delta fibers. The need for 2 or more reasons to initiate root canal treatment

 

ย (Part Three)ย  ย  ย  Post Root Canal Therapy

Participants will learn:

  • what is the purpose of placing a post, is there a difference between cast post, parapost and fiber post
  • coronal seal and restorability, how important is the ferrule
  • when do we need to do crown lengthening, what can happen if the biological width is compromised
  • how to manage post root canal treatment pain/sensitivity to tapping/percussion, when do we place the crown on a root canal treated tooth
  • is there a need to crown the root canal treated tooth.
  • is there a difference in success rate between a first time RCT and a retreatment case
  • what are the treatment options if a RCT fails
  • how to remove fiber post, plastic carrier and gutta percha in retreatment cases
  • walking bleach / internal bleaching, how to take steps to minimise cervical root resorption
  • clinical records

Others:

  • how to manage deep caries with asymptomatic reversible pulpitis. Does giving a local anaesthetic make a difference to managing carious dentine? Do we leave some carious dentine behind vs risk carious pulp exposure?

Backed by the knowledge and insights gained from the lecture, and harnessing the experience shared by Dr Koh Eng Tiong, the hands-on session with Dr Gabriel Tan seeks to incorporate technology and techniques that has made an impact in endodontics into root canal treatment for more predictable results.

This course aims to increase the participantsโ€™ confidence in performing root canal treatment by equipping you with a full overview of root canal treatment from the start to the end, and to gain a better understanding of the tools and techniques that are being used along the way. We will go through a protocol that is safe and touch on ways to reduce the risk of iatrogenic errors so as to perform successful root canal treatment.

Participants can expect to experience the use of heat-treated rotary files and various obturation techniques, including the use of bioceramic sealers. Participants can visualize how these materials and equipment perform in a plastic tooth before moving on to extracted teeth, with ample time to feel how these materials work in your hands for a smoother experience in your journey in endodontics.

1 Day Lecture on 23th November 2024ย 

Dr. Koh Eng Tiongย 

BDS, MSc, FAMS

Dr Koh Eng Tiong obtained his BDS degree from the National University of Singapore in 1984. He practiced as a general dentist for about 10 years before obtaining his MSc degree in Endodontics (with Distinction) in 1995 at the Guyโ€™s Hospital, London. His mentor was the late Prof Tom Pitt-Ford. He did his microscope training at the University of Pennsylvania in 1998, conducted by Prof Syngcuk Kim.

Dr Koh spent more than 3 years at the Endodontic unit of the National Dental Centre, Singapore, before starting his own practice in 2000. He joined Raffles Dental in 2019. Dr Koh is a fellow of the Academy of Medicine (Singapore) since 1999 and in 2003, he was an Examiner to the Conjoint MRDRCS (Edinburgh) โ€“ CDSHK Intermediate Examination in the Specialty of Endodontics in Hong Kong. He is listed on the Roll of Dental Specialist (Singapore).

Dr. Gabriel Tanย 

Dr Gabriel graduated from the National University of Singapore with a Bachelor of Dental Surgery in 2014. He worked as a dental officer at Singhealth polyclinics and National Dental Centre Singapore. It was there that he firmed up his interest in Endodontics and went on to pursue his Master of Dental Surgery in Endodontics at the National University of Singapore. He published his MDS research which compared the effects of AH plus and Bioceramic sealers on post obturation pain in the Journal of Endodontics.

After earning his Master of Dental Surgery, Dr Gabriel continued his career in Endodontics in National Dental Centre Singapore where he completed his advanced specialty training and is recognized by the Singapore Dental Council as a Specialist in Endodontics. During his stint in NDCS, he was put in charge of the dental officers who were doing their advanced general dentistry posting in Endodontics and worked closely with them.

Dr Gabriel is currently practicing in private practice and serves as a visiting specialist at the Singapore Armed Forces. He is also a member of the executive committee of the Society of Endodontists, Singapore.
